Relocating to the Westgate Community Center in West Palm Beach

The Diabetes Education and Research Center, a part of FAU’s Christine E. Lynn College of Nursing, will relocate to a new, centrally-based location at Westgate Community Center, 1650 Osceola Drive; West Palm Beach, FL  33409, on Thursday January 2, 2014. The Westgate Community is located in the heart of West Palm Beach, just north of Okeechobee Blvd., between N. Congress Ave. and N. Military Trail.

Long recognized as a vulnerable and under-served community, the Westgate Community Center location will allow the Diabetes Education and Research Center to further its mission to improve health equity and create healthy communities.

Expanding the Center’s Health Services  and Education

Coinciding with its relocation, the FAU Diabetes Education and Research Center will become a program under the new FAU Community Health Center.   With the vision of improving health equity and creating healthy communities by providing community-based primary care grounded in a philosophy of care, the center will provide a comprehensive and holistic approach to managing health care, helping patients control their health so they can live happy, active and productive lives.

The Community Health Center will maintain its special niche in diabetes care and education for both children and adults, while providing additional services to its patients, including comprehensive primary care, mental health services, well-child services, chronic disease management, nutrition services, laboratory services, referrals to specialty care and case management. The Community Health Center will also offer diabetes, obesity and hypertension classes.
